The Wellbeing Services County of Päijät-Häme – Päijät-Sote
Päijät-Sote adopted Medanets’ mobile solutions in autumn 2017. The contract was awarded to Medanets following a call to tender. The Medanets app is now in use across 26 wards in the region. The features used are Observation values, Monitored patients, Early Warning Score (EWS) and Clinical forms.
Documenting observation values and EWS from the patient’s bedside
In Päijät-Sote, patient parameters are recorded on a mobile app. This way, observation values can be documented from the patient’s bedside, with the data transferred directly to the Electronic Health Record. Similarly, the EWS can be calculated in treatment situations without leaving the room and care staff can refer back to earlier entries on the same mobile app.
Reducing documentation errors with the Monitored patients feature
Using the Monitored patients feature, patient monitoring systems’ data can be transferred wirelessly to the Electronic Health Record, either automatically or once the nurse gives their confirmation. This reduces documentation errors and eliminates duplicate entries. There are approximately 150 patient monitor interfaces in use across the wellbeing services county.
Clinical forms put to good use
The Medanets Clinical forms feature is used in Päijät-Sote for purposes such as prevention of the negative impacts of substance abuse. The human and financial costs associated with excessive consumption of alcohol and other intoxicants are a cause for concern in the area, so that new tools have been sought and adopted to facilitate the work of healthcare professionals dealing with substance abuse. One such tool is AUDIT-C screening.
Traditionally carried out on paper, the AUDIT-C screening questionnaire is now used on mobile, thanks to the Medanets app. At the customer’s request, Medanets also developed a mini-intervention feature to support the screening process. A mini-intervention is a 10–15-minute brief guidance session carried out for patients given a high-risk score. The healthcare professional can confirm in the app that the mini-intervention has been carried out. Hence, the app has helped systemise and structure substance abuse work.
Other clinical forms used on mobile within Päijät-Sote are the Braden scale, FRAT, NRS-2002, CAM, MNA, BDI21, AUDIT65 and GDS30. Medanets Clinical forms is also a key tool on the geriatric ward at the wellbeing service county’s Rehabilitation Hospital Jalmari. The mobile solution has made it easier and faster to carry out a comprehensive assessment of a geriatric patient’s functional capacity.
The Medanets app forms part of Päijät-Sote’s digital strategy, helping it achieve the desired results. Time spent with patients has increased and multidisciplinary work has become more efficient. Healthcare professionals working for the wellbeing services county have found the Medanets app to be clear and easy to use.
